Role Title: Nurse Practitioner, Advanced Practice Provider Company: Oak Street Health Job Description The purpose of a Nurse Practitioner at Oak Street Health is to provide effective and equitable value-based primary care to adults on Medicare to keep them healthy and out of the hospital. Nurse Practitioners see patients independently and collaborate with physicians as needed. They provide longitudinal care, build meaningful patient relationships, and practice within collaborative outpatient teams composed of physicians, nurse practitioners, physician assistants, medical assistants, a dedicated medical scribe, and support from registered nurses and care managers. Core Responsibilities Provision of exceptional primary care. Conduct office visits for routine and acute issues. Administer annual wellness visits and health risk assessments, focusing on holistic health and specific documentation. Care coordination with providers, specialists, testing facilities, and agencies. Population health leadership in coordination with the Care Team (e.g., ensuring eligible females receive evidence‑based breast cancer screening every 2 years). Assisting the care team with phone triage and outreach. Educating patients on their health conditions, care plans, and treatments. Participating in Oak Street Health promotional activities. Conducting home visits as needed. Other duties, as assigned. Required Qualifications Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate. National certification in at least one specialty: Family NP, Adult‑Gerontology NP, Primary Care NP, Adult NP, or Gerontological NP. Active, non‑probationary state Nurse Practitioner license. Active DEA license. U.S. work authorization. Preferred Qualifications Experience in primary care: internal medicine, geriatrics, or family medicine. Knowledge of HCC (Hierarchical Condition Categories) documentation, ICD‑10 coding, and Health Risk Assessments (HRAs). Passion for teamwork and cross‑functional collaboration. Desire to be part of an innovative model focused on empirically guided population health. Bilingual proficiency in applicable areas. Compensation Anticipated weekly hours: 40 Time type: Full time Pay Range: $87,035.00 – $187,460.00 (based on experience, education, geography, and other factors).
