Therefore, final offer amounts may vary from the amount stated.Job Responsibilities & Essential FunctionsValid driver’s license and reliable transportationAbility to transport clients in your own personal vehicleMust be 21 years of age or olderExperience working with transitional age youthAbility to work independentlyGreat organizational and communication skillsWillingness to work a flexible schedule as needed to include occasional weekend and/or evening workSupporting, coaching, and training the youth in age- appropriate behaviors, interpersonal communication, problem-solving and conflict resolution, and relating appropriately to other children and adolescents, as well as adults, in recreational and social activities pursuant to a behavioral health treatment plan developed by an outpatient providerEngaging the youth in activities in the communityAddress one or more goals each session on a youth’s existing outpatient or In-home Therapy treatment plan (for youth not in ICC), or on an existing ICP (for youth in ICC)Carry an average caseload of 3-5 if part-time, or 8-12 if full-time, of which she/he will be responsible for case managementDevelop an individualized service plan with the clinicianResponsible for monthly/quarterly summaries, progress notes, and discharge paperwork.
