Role Description The Care Manager is a full-time, on-site role located in Glen Cove, NY, responsible for supporting residents through high-quality, person-centered care. Day-to-day responsibilities include coordinating and monitoring care plans, assisting with activities of daily living, and supporting residents’ physical, emotional, and social needs. The Care Manager collaborates with clinical teams, families, and other service providers to ensure continuity of care and timely communication. The role also involves observing and reporting changes in residents’ conditions, promoting engagement in programs and activities, and upholding Sunrise’s standards for safety, dignity, and respect. Care Managers contribute to a positive community environment by modeling compassionate service and supporting new team members with guidance and training. Qualifications Candidates should possess strong Case Management skills, including care coordination, care plan implementation, and follow-up. Candidates should possess Training and mentoring abilities to support onboarding and ongoing development of care team members. Candidates should possess Elder Care experience, including support with daily living activities and awareness of age-related needs. Candidates should possess familiarity with Social Services resources and processes to connect residents and families with appropriate support. Candidates should possess basic Medicine-related knowledge, such as understanding common senior health conditions and following clinical instructions. Additional beneficial qualifications include prior experience in senior living or healthcare settings, strong communication and interpersonal skills, and the ability to work effectively in a team-oriented, fast-paced environment. Relevant certifications such as CNA, HHA, or other caregiving credentials, and CPR/First Aid certification, are preferred but not required depending on community needs. #J-18808-Ljbffr Sunrise Senior Living
