RN, Community Engagement & Care Coordination (ACT I)

Care New England Health System is seeking a Registered Nurse for the ACT I Engagement and Diversion Team in Providence, RI. This part-time role requires assessing health needs and coordinating care for clients with mental health and substance use issues. The Nurse will engage with clients both in the office and community settings, and ensure high-quality, person-centered services. Responsibilities include nursing assessments, developing care plans, and managing crisis situations. Candidates should have a valid RN license in RI and relevant experience is preferred.
