Licensed Practical Nurse (LPN) – Nurse Clinic (Outpatient)The Licensed Practical Nurse (LPN) provides direct patient care in a nurse clinic within an outpatient setting. The LPN performs delegated nursing tasks and protocol-based care under the supervision of a Registered Nurse (RN) or licensed provider, in accordance with Oregon State Board of Nursing guidelines. Primary responsibilities include lab draws, immunizations, wound care, and other task-oriented clinical services.Major duties and responsibilities include:Perform patient intake, including vital signs, allergies, and medication reconciliationConduct venipuncture and capillary blood collection for ordered laboratory testingProcess, label, and prepare specimens for transport in accordance with lab protocolsAdminister medications and injections, including vaccines, per provider orders and approved standing ordersMonitor patients for immediate adverse reactions following medication or vaccine administrationProvide immunizations in accordance with clinic protocols, standing orders, and state guidelinesPerform wound care, including dressing changes, basic wound assessment, and documentation of healing progressConduct ear lavage, simple treatments, and other approved clinical proceduresPerform point-of-care testing (e.g., glucose, A1c, urine testing) per protocolReinforce provider-directed patient education related to treatments, medications, and follow-up carePractice under the direction of an RN or licensed independent provider as required by the Oregon State Board of NursingPerform only those duties that are within LPN scope and for which competency has been validatedFollow established standing orders, clinical protocols, and delegated tasksReport patient status, abnormal findings, and concerns to the supervising RN or provider promptlyDo not perform independent nursing assessments, triage, or care planning beyond LPN scopeAccurately document all care, procedures, and patient interactions in the electronic health record (EHR)Maintain compliance with infection control standards, including proper handling of sharps and biohazard materialsEnsure patient confidentiality in accordance with HIPAA regulationsFollow medication administration safety standards, including the "Five Rights"Maintain clean, stocked, and organized nurse clinic and treatment areasEnsure proper storage and handling of vaccines, including cold chain managementMonitor and restock clinical supplies as neededParticipate in competency validation and ongoing skills trainingLike all employees of the Klamath Tribes, the incumbent will be called upon to accomplish other tasks that may not be directly related to this position, but are integral to the Klamath Tribes' broader functions, including but not limited to, assisting during Tribal sponsored cultural, traditional, or community events that enable the successful operation of programs and practices of The Klamath Tribes as aligned with The Klamath Tribes' Mission Statement. Some of these tasks may be scheduled outside of regular work hours, if necessary.Knowledge, skills, abilities:Ability to maintain strict confidentiality of medical records and adhere to the standards for health record-keeping, HIPAA and Privacy Act requirements.Proficiency in venipuncture and specimen handlingCompetence in vaccine administration and injection techniquesKnowledge of basic wound care and infection prevention practicesUnderstanding of LPN scope of practice and delegation requirements in OregonAbility to follow established protocols and standing orders with accuracyStrong documentation and organizational skillsEffective communication with patients and healthcare team membersQualifications, experience, education:REQUIRED to possess a High School Diploma or Equivalent. (Must submit copy of diploma or transcripts with application.)REQUIRED Current Licensed Practical Nurse (LPN) license in the state of OregonREQUIRED Current Basic Life Support (BLS) certificationREQUIRED Graduate of an accredited practical nursing programFor out of state applicants; Oregon Licensure required within 90 days of hire. (Must submit copy of Licensure with application.)REQUIRED to possess and maintain a valid Oregon Driver's License, (out of state applicants must receive ODL within 90 days of hire), have good driving record and be insurable by the Klamath Tribal Health & Family Services' vehicle insurance policy. (Must submit copy of driver license with application.)REQUIRED to submit to a background and character investigation, as per Tribal policy. Following hire must immediately report to Human Resource any citation, arrest, conviction for a misdemeanor or felony crime.REQUIRED to submit to annual TB skin testing and adhere to KTHFS staff immunization policy in accordance with the Centers for Disease Control immunization recommendations for healthcare workers.REQUIRED to acquire and maintain BLS certification within 180 days of hire; strongly recommend certification within 90 days of hire, probationary period will be extended for a maximum of 180 days pending certification.REQUIRED to acquire and maintain ACLS certification within 180 days of hire; strongly recommend certification within 90 days of hire, probationary period will be extended for a maximum of 180 days pending certification.REQUIRED to accept the responsibility of a mandatory reporter in accordance with the Klamath Tribes Juvenile Ordinance Title 2, Chapter 15.64 and General Council Resolution #2005 003, all Tribal staff are considered mandatory reporters.REQUIRED to be free from exclusion from providing Federal health care benefits including Medicare and Medicaid as per the Federal OIG and GSA exclusion lists.Preferred Qualifications:Previous experience in outpatient, primary care, or nurse clinic settingExperience with high-volume immunization clinics and lab drawsFamiliarity with EHR systemsIndian Preference:Indian and Tribal Preference will apply, as per policy.
