Job Summary:

The Physical Therapist evaluates, plans, and implements patient rehabilitation programs to improve mobility, strength, and function. They oversee treatment processes, supervise assistants, and ensure delivery of high-quality care in an outpatient rehabilitation setting.

Responsibilities:

• 
  Assess patient conditions and develop individualized physical therapy treatment plans.
  
  
• 
  Utilize Electronic Medical Records to document evaluations, progress notes, and discharge summaries.
  
  
• 
  Supervise and guide PT Assistants and Aides in delivering patient care.
  
  
• 
  Communicate effectively with physicians, team members, and patients to coordinate care and address concerns.
  
  
• 
  Ensure compliance with HIPAA, billing, and coding policies, including ICD-10 and DX.
  
  
• 
  Maintain necessary licensure, certifications, and stay current with industry standards and protocols.
  
  

Qualifications:

• 
  Hold an active Physical Therapy license in the state of practice or plan to obtain it soon.
  
  
• 
  Possess a degree in Physical Therapy from an accredited program.
  
  
• 
  Demonstrate proficiency in EMR systems, electronic documentation, and ICD-10 coding.
  
  
• 
  Exhibit strong communication, decision-making, and interpersonal skills.
  
  
• 
  Be physically capable of standing, walking, lifting up to 50 pounds, and assisting patient mobility.
