This role offers the rare opportunity to be a cornerstone in launching Emmanuel Hospice’s new palliative care program, shaping its vision, structure, and long-term impact. As a key leader, you will guide the development of innovative care models that will meaningfully support patients who are facing serious illness and their families. Your expertise will directly influence how the program grows, evolves, and becomes an essential part of the Emmanuel Hospice continuum of care.Reporting directly to the CEO, the Palliative Care Manager ensures the program reflects excellence, regulatory compliance, and the mission to support patients and families with serious illness.Key Responsibilities:Lead all program startup activities, including workflows, policies, and operational designManage program budget, financial performanceSupervise and support palliative care staff in collaboration with the Medical DirectorOversee compliance with Medicare, Medicaid, commercial payers, and accreditation standardsSupport provider credentialing and payer contractingEnsure efficient scheduling, referrals, documentation, and care transitionsDrive program growth, customer service, and performance improvementDeliver social work services during start up and as neededParticipate in the Manager On-Call Rotation for hospice servicesQualifications:Master's degree in social work required; hospice or palliative care related leadership experience preferred.
