Social Worker - MSW Required

Provides and coordinates social services including psychosocial support to patients and families Performs psychosocial assessments and empowers clients Consults with clinical staff to address family dynamics related to health problems Maintains documentation reflecting timeliness and accuracy Participates in developing individualized Plans of Care and attends Case Conferences Requirements Master’s degree or above in Social Work (CSWE accredited) Minimum of one (1) year of experience including social work/counseling internship in health care or hospital setting Familiarity with adult and pediatric patients' concepts Previous experience with an EMR/EHR system Mobile Driver - Valid driver's license and automobile insurance Ability to physically access homes and facilities Core Competencies Demonstrates expertise in providing psychosocial support and conducting assessments for patients and families in healthcare settings. Proficient in developing individualized Plans of Care and collaborating with clinical staff to address family dynamics.
