Intensive Field Case Manager — Housing & Health Services
Upward Bound House
Upward Bound House is seeking an experienced Intensive Case Manager to join our team delivering field-based ICMS to homeless families in Los Angeles. You will work with a multidisciplinary team, engage participants in housing stability plans, and coordinate access to health and social services in the community and in participants' homes. Responsibilities include outreach, assessment, service planning, benefits and subsidy applications, housing location, and collaboration with DHS and FSC #J-18808-Ljbffr Upward Bound House
