LTSS Service Care Manager – LCSW Pay Rate: $34.50/hour Location: Remote – North Carolina, Region 4 Preferred Areas: Durham & Wake Counties Schedule: Monday–Friday, 8:00 AM–5:00 PM EST Contract: 5 months with potential extension or conversion Travel: 50%+ field travel Position Overview We are seeking an experienced Licensed Clinical Social Worker (LCSW) to join our care management team as an LTSS Service Care Manager. This position combines remote care management with face-to-face visits with members in their homes and communities. You will assess member needs, develop and monitor individualized service plans, coordinate healthcare and community resources, and work with members, families, caregivers, and providers to ensure appropriate services are available. This is an excellent opportunity for an LCSW with experience in in-home services, long-term care, LTSS, HCBS, care coordination, or community-based social work who is comfortable working independently in a field-based environment. What You'll Do Conduct assessments to identify member needs, available resources, and barriers to care. Develop and monitor individualized long-term care and service plans. Coordinate services between members, families, caregivers, healthcare providers, specialists, and community organizations. Conduct face-to-face home and community visits with members. Monitor member status, services, care plans, and outcomes. Identify appropriate healthcare, social-service, and community resources. Help members access services and benefits needed to support their health and independence. Provide education to members, families, and caregivers regarding available services, benefits, referrals, and care options. Coordinate with providers and care-management partners to address gaps in services. Support transitions and ongoing coordination across the healthcare continuum. Maintain accurate assessments, care plans, case notes, and other required documentation. Use electronic systems and Microsoft Office tools to document and manage member information. Participate in virtual team meetings and required training. Work independently while managing an assigned member caseload. Required Qualifications Active LCSW license OR RN license required. Bachelor's degree required. At least 2 years of care management or in-home clinical experience. Experience with LTSS, HCBS coordination, care management, or care delivery monitoring. Experience conducting assessments and developing or monitoring care plans. Strong clinical documentation skills. Strong computer skills and ability to learn electronic systems. Ability to work independently and manage a field-based caseload. Valid driver's license. Reliable personal transportation. Ability to travel regularly for face-to-face member visits. You will perform remote administrative and care-management activities while also traveling throughout your assigned North Carolina service area for member visits. Candidates should be comfortable: Conducting face-to-face home visits. Driving throughout their assigned territory. Managing multiple member visits and appointments. Working independently outside of a traditional office environment. Completing documentation electronically following member interactions. Training Initial training is conducted remotely Monday–Friday, 8:00 AM–5:00 PM EST. Full participation during the training period is required.
