Community Outreach & Partnerships Coordinator

Sun River Health

Sun River Health is seeking a full-time Community Outreach Coordinator I for our Amenia site. The role focuses on developing long-term relationships with external referral sources and coordinating with internal HRHCare programs to improve access to services for at‑risk clients. The coordinator will manage 15–25 organizational relationships, conduct about 40 visits per month, and report to the Regional Manager of Community Engagement.
