Inpatient Care Manager & Discharge Navigator
St. Luke’s University Health Network
St. Luke’s University Health Network is seeking a Social Worker or Registered Nurse Care Manager to coordinate complex discharge planning and provide psychosocial assessment and interventions. You will work with physicians, social workers and care teams to achieve desired patient outcomes and reduce readmissions.
