Geriatrician

Physician - Geriatric Medicine Profession: Physician Specialty: Internal Medicine - Geriatric Medicine Duration: Temporary Coverage Shift: 4 Days a Week Hours per Shift: 10 hours Experience: Board-certified or board-eligible in Internal Medicine or Family Medicine with experience in Skilled Nursing, Long-Term Care, Transitional Care, and post-acute medicine. Experience with geriatric and medically complex patients is strongly preferred. License: Must hold a valid medical license. Certifications: ACLS, BLS required. Must-Have: Experience in managing skilled nursing and long-term care residents. Description:Provider will have a dedicated Advanced Practice Provider.Schedule will be three days a week, preferably Tuesday, Wednesday, and Thursday.The shift is from 7 a.m. to 5 p.m. No call is required.Clientists will cover night call.Temporary physician coverage is needed for the Skilled Nursing Facility and Long-Term Care unit.The physician will provide comprehensive medical oversight for both short-term rehabilitation patients and long-term care residents.Responsibilities include ensuring regulatory compliance, continuity of care, and timely medical management during a provider vacancy.Review and approve admissions to the Skilled Nursing Facility, conducting comprehensive history and physical examinations for all new admissions.Conduct required physician compliance and recertification visits according to regulatory requirements for both Skilled Nursing Facility and Long-Term Care residents.Provide ongoing primary medical care for long-term care residents, managing chronic medical conditions and preventive care.Develop, review, and update individualized care plans in collaboration with nursing, rehabilitation services, case management, and social work.Manage acute changes in condition, evaluate new symptoms, and provide treatment to reduce unnecessary emergency department transfers and hospital readmissions.Prescribe, monitor, and adjust medications while reviewing consultant pharmacist recommendations and ensuring medication safety.Participate in interdisciplinary care conferences, family meetings, and goals-of-care discussions as appropriate.Coordinate transitions of care between the hospital, skilled nursing, long-term care, specialty services, and community providers.In addition to skilled nursing patients receiving post-acute rehabilitation, provide ongoing medical oversight for long-term care residents who require continuous physician management.Perform routine medical evaluations and chronic disease management.Conduct annual wellness and preventive care assessments.Manage dementia, frailty, and other complex chronic conditions common in the long-term care population.Oversee palliative and comfort-focused care plans when appropriate.Collaborate with families and healthcare decision-makers regarding changes in condition, treatment plans, and advance care planning.Certification and documentation are required to maintain residents' long-term care plans and regulatory compliance.Patient encounters are expected to be between 12-14 per shift.Nearest airport is Glacier International Airport.Lodging recommendations are available upon request.This facility operates at trauma level 3.Position requires a minimum board certification.Estimated credentialing timeframe is 120 days, with temporary/emergency privileges available.Certain certifications such as ABLS, PALS, and ATLS are not required. #J-18808-Ljbffr
