Physical Therapist - Travel Contract

Looking for a change of scenery and a chance to help others. Join a travel Physical Therapist opportunity in Escanaba, MI providing home health services. You will help patients recover from injuries and manage chronic conditions while working within a home health practice and covering Delta County and portions of Dickinson and Menominee Counties within approximately a 1-hour radius of Escanaba. Minimum Qualifications Doctoral, Master’s, or Bachelor’s degree in Physical Therapy from an accredited program approved by the APTA. Active Michigan Physical Therapist license required to start the assignment. Home health experience required. BLS certification from AHA or ARC may be required. Ability to complete physical therapy admissions, evaluations, reassessments, and discharges in a home health setting. Ability to create individualized plans of care and supervise/work with a PTA. Preferred Qualifications Experience with HCHB (Homecare Homebase). Good navigation skills and ability to follow GPS directions. Wound care or lymphedema certification. Assignment Details Shift hours: 0800 - 1600 (office hours noted as 8:00 AM - 4:30 PM; clinicians self-schedule). Workweek: Monday through Friday, office-hour schedule. Weekly hour guarantee: 32 hours. Productivity expectation: 30 points weekly. Coverage area: All of Delta County and portions of Dickinson and Menominee Counties within a 1-hour radius of Escanaba. Mileage reimbursed between visits from first patient to last patient. No on-call or weekend requirement. Dress code: Navy blue scrubs or navy blue polo with khaki, black, or navy slacks. Impacting Patient Care Provide comprehensive home health physical therapy services including admissions, add-on evaluations, reassessments, discharges, individualized plan-of-care development, and collaboration with PTAs. Participate in weekly case conferences and monthly staff meetings to support care coordination and quality outcomes.
