Overview The Physical Therapist in Home Health performs assessments and evaluations of patients’ functional status, mobility, endurance, pain, and other medical conditions. Based on these findings, the PT develops treatment plans, performs interventions to improve patients’ well‑being, and evaluates progress. Primary Responsibilities Evaluate patients’ functional status and physical therapy needs. Assist the physician in developing the therapy plan of care. Provide services within scope of practice as defined by state laws, following the plan of care and coordinating with other health care team members. Observe and report patients’ responses to treatment and any changes in condition to the supervising nurse and/or physician. Treat patients to relieve pain, restore function, and/or maintain maximum functioning using acceptable standards of practice. Instruct and inform members of the health care team, patients, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals. Visit patients per the Plan of Treatment, complete a progress note for each visit, and submit progress notes per LHC policy. Complete all patient evaluations and develop the PT plan of care within state-specific guidelines. Report outcomes of evaluation, goals, and anticipated projected frequency of care. Required Qualifications Current Physical Therapy licensure in state of practice. Current CPR certification. Current driver’s license and vehicle insurance; access to a dependable vehicle or public transportation. Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client. Pass a required drug test before beginning employment. Compensation & Benefits Includes a $10,000 sign‑on bonus for external candidates. The hourly pay for this role ranges from $34.23 to $61.15 per hour based on full‑time employment.
