Community Health Guide: Field Outreach & Support
Reema Health
Reema Health in Cleveland, OH seeks a Community Health Navigator (Community Guide) to build trusted relationships with members and guide them toward engaged, healthier lives. You will work with a multidisciplinary team to address social barriers and document progress using Reema's platform. The role requires 80–120 member caseload management, daily outreach, and strong communication skills.
