Field-Based Behavioral Health Guide & Outreach
Reema Health
Reema Health is seeking a Community Guide to work with members facing serious mental illness and substance use disorders in the Cleveland area. You will build relationships, coordinate services with the care team, and document all interactions using our integrated platform. The role requires 60–100 member caseload management, field visits, and collaboration with clinicians to support enrollment and ongoing care.
