Conduct thorough evaluations for patients in both inpatient and outpatient settings, crafting personalized treatment plans based on individual needs. Implement targeted physical therapy interventions enhancing mobility, strength, balance, and overall function. Collaborate effectively with nurses, physicians, case managers, and other rehabilitation professionals to achieve optimal patient outcomes. Educate patients and their families about therapeutic goals, discharge planning, and at-home exercise routines. Maintain accurate documentation of evaluations, treatments, and patient progress in accordance with facility and regulatory standards. Engage in care team rounds and interdisciplinary conferences to facilitate coordinated patient care.

Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution. Valid state licensure as a Physical Therapist. CPR certification preferred. Additional specialized certifications (e.g., neurological, geriatric) are favored but not mandatory. Previous experience preferred, particularly in treating complex medical and mobility challenges. Excellent communication and teamwork abilities. Proficient with digital charting and clinical documentation systems.
