This advertisement may be used to fill multiple positions. Your Specific Responsibilities Perform direct HIV/STI counseling and testing services on inmates submitting and consenting to have such tests performed in accordance with Florida Department of Health HIV/STI testing Guidelines and processes specimen collections for submission to the State Laboratory. Perform direct court‑ordered HIV/STI/HCV counseling and testing services when required. Act as liaison between the Florida Department of Health, St. Lucie County, St. Lucie County Jail, Department of Juvenile Justice, and other detention facility officials and subcontractors to coordinate services and ensure Jail Linkage Programmatic Services are delivered at the highest level of quality. Attend all service‑coordination meetings and maintain an up‑to‑date contact list of detention center and subcontractor personnel. Facilitate regular HIV/STI/HCV prevention education classes for inmates, distribute and collect evaluation forms and pre‑ and post‑tests, award certificates of completion, and complete monthly and quarterly reports accurately. Complete required case management forms, laboratory request forms, travel reimbursement forms, and other records in accordance with CHD, HIV/AIDS Linkage to Care – Prevention Program, and STD program policy. Perform case‑management activities to identify individuals infected with STI, hepatitis, HIV/AIDS, or other agents, including post‑test counseling, partner counseling, testing, referral services, and data entry into Linkage Module/STARS. On confirmation of HIV/STI/HCV positive results, conduct original post‑test counseling interviews, partner services, testing, referral services, and link individuals to the care system both internally and externally in collaboration with the Linkage to Care Program. Communicate with public and private clinics, hospitals, laboratories, and other health-care providers to obtain client information and arrange medical evaluations and treatments. Assure that newly diagnosed clients are linked to care within 30 days, facilitate re‑engagement of previously diagnosed clients, and assist providers in locating non‑compliant clients. Identify barriers to care and assist HIV‑positive individuals with referrals, transportation, medication, Medicaid or insurance as needed. Perform outreach events, community services, and rapid testing using venipuncture or other techniques at community events, detention facilities, and DOH clinics. Prepare and maintain timely and accurate records and reports according to program guidelines and requirements. Foster and initiate a client‑led support group for interested clients and assist with recruitment. Develop appropriate materials, resources, and processes to improve outcomes and efficiency of quality care for HIV‑positive clients, high‑risk clients, CBOs, and LTC staff, and disseminate information on trainings. Maintain client confidentiality and security in accordance with DOHP 50‑10 Information Security & Privacy Policy, certify and submit EARs, cell phone verification, Everbridge alerts, CSRs, and timesheets per DOH policy. Participate in department and unit meetings, provide feedback to supervisors and providers, and perform other related duties as assigned. Required Knowledge, Skills, and Abilities Ability to travel in and out of county within area 15 and possess a valid driver’s license. Case‑manager experience and phlebotomy training. Self‑motivated, flexible, and capable of initiating tasks. Strong presentation skills in PowerPoint and effective communication. Exceptional interpersonal, organizational, and time‑management skills, and ability to work in teams. Experience completing Linkage to Care (ARTAS) and Disease Intervention Specialist training, delegating tasks, and interacting comfortably with individuals and groups. Knowledge of diverse communities and ability to plan and coordinate work assignments. Proficiency in Microsoft Office (Word, PowerPoint, Excel, Outlook). Qualifications Minimum Willingness to become certified in phlebotomy and complete Disease Intervention Specialist and Linkage to Care ARTAS training. Willingness to travel out of county, including undesirable neighborhoods and/or between counties. Willingness to work before and after hours, on weekends, and in special‑needs shelters during hurricanes or other emergencies. Minimum one‑year case‑management experience and/or patient‑care coordination/navigation experience. Willingness to provide services to all residents in detention facilities/jails, rehabilitation/drug addiction facilities, mental health facilities, and subcontractors in Area 15. Valid driver’s license and completion of required background screening, including fingerprinting and criminal‑history checks. Preferred Certified phlebotomist or trained phlebotomist. Experience working with HIV/STD programs or related work. Experience performing quality assurance, file reviews, reports, and process development. Experience with detention facilities, drug rehab facilities, mental health facilities, and related work. Proficiency with Microsoft Word, Excel, PowerPoint, and other related programs. Case‑management experience or experience with caseloads. Experience providing risk‑reduction education on topics such as HIV, STDs, safe sex practices, and conducting community events. Experience working with marginalized, low‑income, and disenfranchised communities. Bilingual (Spanish and/or Creole) preferred. Benefits Annual and sick leave benefits.
