This posting could be used to fill future vacancies This posting may fill multiple positions Responsibilities This position provides medical case management to persons infected and affected by HIV/AIDS within the four‑county area of Indian River, Martin, Okeechobee, and St. Lucie Counties (area 15). Proactive, inclusive case management activities are conducted through office or field visits according to Florida Department of Health guidelines. Key duties include: Comprehensive needs assessments and acuity assessments within specified timeframes. Development and implementation of individualized care plans, with ongoing monitoring and re‑evaluation. Documentation of all client interactions and case notes in CAREWare and HMS databases within three business days. Provision of counseling, education, and medication adherence support; monitoring of HIV medication therapy and laboratory values. Linkage, referral, and patient navigation to community and government services; authorization of transportation where needed. Participation in conference calls, staff meetings, and mandatory trainings; submission of required reports and timesheets. Required Knowledge, Skills, and Abilities Knowledge of effective interviewing and communication techniques. Knowledge of English language structure, composition, and grammar. Detail‑oriented, independent judgment, and ability to work under stress. Ability to work with a diverse population and maintain client confidentiality. Ability to prepare correspondence, reports, and organize work assignments. Effective verbal and written communication; ability to prioritize workload. Qualifications Minimum of one (1) year of experience in a social service‑related agency. Willingness to work before, during, or beyond normal hours for emergencies, including disaster response or Red Cross shelters.
