The Asian American Health Initiative (AAHI) is currently recruiting an enthusiastic professional to serve as the Senior Wellness Coordinator. The Senior Wellness Coordinator is responsible for raising awareness of the health and wellness concerns of Asian American seniors and facilitating community organizations to develop programming to address these concerns. The Senior Wellness Program Coordinator will play a significant role in the development, coordination, implementation and evaluation of the deliverables of senior programming that lead to improved health, wellness, and quality of life. Design, implement, and evaluate health education initiatives and events for AAPI seniors using PDSA cycle. Plans, organizes, coordinates, and evaluates complex program activities that are culturally and linguistically competent, which involves making each program multilingual in English, Chinese, Korean, Vietnamese, Hindi, and other Asian languages. Coordinates and facilitates multilingual educational programs for seniors regarding health, safety and resources. Educates seniors and caregivers on available resources and application procedures. Conducts outreach to seniors and caregivers. Develops and maintains a multilingual database of senior groups and resources. Collects, analyzes, and evaluates data and information with the goal of making appropriate recommendations for programs. Lead the work for senior wellness contractors aimed at reducing senior isolation and improving health outcomes Provides multilingual interpretation and translation services to ensure effective communication with LEP seniors and caregivers. Active involvement in relevant workgroups, committees, and in department-wide projects Liaises with providers and stakeholders. Establishes and maintains effective working relationships with a variety of individuals and groups. Collaborates with HHS service areas and programs, including Aging and Disability Services. Represents AAHI on workgroups, committees or coalitions when available. Participates in department-wide initiatives and projects. Facilitates multilingual communication in meetings and collaborations to ensure inclusivity and understanding. Content Development and Training Creates innovative and creative culturally and linguistically competent education materials. Develops, delivers, and evaluates cultural responsiveness trainings for healthcare providers to improve service delivery to AAPI clients. Utilizes innovative and creative multilingual health education methods to educate hard-to-reach members of the Asian American community. Research and create new training curricula as needed. Develops multilingual training materials and conducts training sessions in multiple languages to cater to diverse audiences. Supports administrative services Provides support on the preparation of special events and meetings. Provides support with contract monitoring. Develop scopes of work for contracts. Represent the department at community events, advisory boards, workgroups, stakeholder meetings, etc. The selected candidate will be required to work evenings and weekend when the needed. This position requires a driver’s license as the selected candidate will travel throughout the County to perform job duties. Experience Two (2) years of professional experience planning and coordinating community health programs that addresses disparities for seniors in the Asian American or Pacific Islander community, focused on health equity, social determinants of health (SDOH), community engagement, and implementing programs that improve health outcomes. Education Graduation from an accredited college or university with a Bachelor's Degree. Driver's License Possession and maintenance at all times of a valid driver’s license from the applicant's state of residence. Equivalency An equivalent combination of education and experience may be substituted. The applications of those individuals meeting the minimum qualifications will be reviewed to determine the extent, relevancy of training, and experience in the following areas: Experience in public health program design, implementation and evaluation using the Plan Do Study Act (PDSA) cycle for Asian communities. Advanced Multilingual proficiency in at least one Asian language such as Vietnamese, Korean, Chinese, Tagalog, Hindi, Urdu, Sinhala, etc. Master’s degree in Aging, Gerontology, public health, social worker Clinical Health Sciences. LCSW-C, CHES, MCHES, or LPC certification. Experience working collaboratively with different partners from the government, business, nonprofit, faith-based, and advisory committees delivering community health education.
