Overview Job title: Case Manager Group: Programs Department: Supportive Services for Veteran Families (SSVF) Region: NC Location: NC regional offices (Durham, Fayetteville, Rocky Mount, or Salisbury) Classification: Non-Exempt Status: Full Time Reports to: Team Lead Supervises: N / A Travel: Telework capacity and travel in the areas of MD, VA, DC and the Carolinas as needed. Summary Provide day-to-day support and services for participating veteran households. Has relevant experience in homelessness prevention, rapid re-housing and transitional supportive services. Provide key linkages for clients from intake and assessment through attainment or maintenance of permanent housing in their home community. Promote accessibility and outreach to targeted populations; provide case management, advocacy and wraparound supports with the VA and in the community. Responsibilities Coordinate outreach efforts for the SSVF Program, including developing an outreach plan to target the three categories of participants and visiting VA facilities, shelters, and other places where homeless and low-income veteran families congregate. Screen potential program participants for eligibility using VA-approved forms and verify information with applicable agencies (veteran status and income). Conduct housing and comprehensive supportive services assessments and develop a comprehensive Family Services Plan. Provide service coordination between the program and community partners for persons served and their families. Collaborate with participants, family members, housing agencies, and professionals to revise service plans and develop placement, stabilization and follow-up plans. Provide or arrange housing support and wraparound services, connecting to VA and mainstream resources to support transition to or maintenance of permanent housing. Travel to housing sites to support participants, monitor progress, and address health, behavioral, or life-skills issues. Communicate effectively (within confidentiality rules) with the clients service team to address and correct issues with services or supports, especially living skills, employment, education, family and socialization. Ensure appropriate use and documentation of any temporary financial assistance. Ensure veterans and their families receive services that support rapid re-housing; address assessed needs, promote healthy decision-making, and support financial independence and community integration. Complete participant category reassessments according to specific SSVF requirements. Participate in program and professional supervision, attend required staff and Project Management Team meetings and trainings, and contribute to team-based planning. Complete all appropriate reporting in HMIS and other systems as required. Complete other duties as needed and requested. Qualifications A bachelor’s degree in counseling psychology, rehabilitative counseling, or related human service field is preferred (or comparable experience). Experience working with homeless individuals and families, including veterans, is a plus. Valid driver’s license in the jurisdiction of residence and vehicle liability coverage if driving a personal vehicle. Privileged to drive according to Agency MVR guidelines. Possess: acceptable criminal background check, OIG exclusion check, negative drug screening, and CPR/First Aid Certification (to be acquired and maintained once employed). The physical requirements include safe operation of a motor vehicle for transportation, ability to travel to assigned locations, stand, bend, reach, lift, and operate office equipment; ability to remain seated for extended periods.
