You’ll typically work at one facility per day, supported by a dedicated care management team that knows your patients and handles the busywork—so you can focus on clinical judgment and relationships. Your week: ~3–4 days in the field, ~1–2 days on telemedicine and care coordination (about 30–50% virtual/flexible work). What You’ll Do Conduct comprehensive assessments and build individualized care plans Collaborate with care managers, physicians, and facility teams Educate and support patients and families Monitor progress and adjust treatment plans Travel to facilities within 30 miles of Seattle What You Bring Active WA Nurse Practitioner or Physician Assistant license Deep compassion and care for patients; willingness to work closely and collaborate with other members of the team Senior care / home health experience preferred (assisted living a big plus) Strong primary care and care management knowledge Excellent communication and ability to work independently, with excellent clinical judgement The Package $145,000–165,000 + $5–10,000 sign‑on bonus CME allowance of 3 days and $3,000 + malpractice coverage Benefits 401(k) Health insurance Paid time off Vision insurance #J-18808-Ljbffr
