Orthopedic At‑Home Physical Therapist – Per Diem (Staten Island) JAG Physical Therapy is seeking a skilled Physical Therapist to provide orthopedic rehabilitation in patients’ homes after total hip and knee replacement surgeries and other related conditions. The role focuses on helping patients regain mobility, strength, function, and confidence in a home setting, with the goal of transitioning care to one of our outpatient sites. Qualifications Bachelor’s or Master’s Degree required; Doctorate preferred Licensed Physical Therapist in the state practicing in Completed state‑regulated CEU requirement in appropriate state (NY, NJ) Valid CPR license Experience/knowledge with an EMR software is preferred Excellent oral and written communication Strong clinical decision‑making skills Excellent work ethic and dedication to patient success Basic computer skills Works as a team member and individually with minimal supervision Maintains a positive, professional demeanor at all times Strong orthopedic background Responsibilities Educate patients on safe bed mobility, transfers, and ambulation respecting post‑operative precautions Transfer, ambulate, and perform stair negotiation with patients to confirm their safety for discharge to home Collect and record patient care data following format and guidelines of the care service, regulatory agencies, and third‑party payers, reflecting an understanding of reimbursement systems Provide initial evaluations, daily notes, reassessments/progress reports, discharge plans, and communicate with team members Complete daily billing and follow department guidelines for productivityPrepare evaluations in accordance with department policies, procedures, and standards of practice, including patient history and diagnosis research Select and administer tests and measures; synthesize evaluation results to formulate diagnosis, goals, prognosis, and plan of care Establish treatment plans based on evaluation results and goals; collaborate with interdisciplinary team and patients’ support system; assess and re‑assess need for modifications Provide education and training to patients and their support system Lead functional training including ADLs/IADLs, therapeutic activities, rehabilitation equipment prescription, and community reintegration Responsible for treatment, plan of care, and exercise prescription for patient caseload Deliver interventions that are safe, effective, and in compliance with JAG standards Document data and write notes using Prompt EMR for all treatments provided Attend annual JAG Billing and Coding seminar Communicate with Manager/Patient Access team for scheduling evaluations after demographics are received Equal Opportunity Employer This employer is required to notify all applicants of their rights pursuant to federal employment laws.
