Shifts Per Week: 4. Shift Duration: 8 Hour Days. Coverage Area: Washington, including King County and Snohomish County. Travel Requirement: More than 50%. Job Requirements 3 years of patient care experience preferred. Active, unrestricted Nurse Practitioner license in the state of Washington. Experience in primary care, adult care, geriatric care, home-based care, or risk assessment preferred. Previous in-home risk assessment experience preferred. Ability to work independently and efficiently in the field. Comfort with EMR documentation and technology-based workflows. Reliable transportation for travel to patient homes within the assigned region. Strong communication skills and a patient-centered approach. Bilingual skills are a plus. Responsibilities Conduct annual in-home wellness visits and comprehensive health assessments for at-risk members. Perform physical examinations, body system reviews, vital signs collection, and medication reconciliation. Evaluate functional status, pain, cognition, mental health, fall risk, and home safety. Document findings accurately to support risk adjustment coding and value-based care initiatives. Provide patient education related to medications, chronic conditions, and wellness goals. Help identify and close quality care gaps, including recommended screenings and labs. Build trust with patients and families through empathy, professionalism, and clear communication.
