Social Worker I

Job Title: Social Worker (Full‑Time) Location: Bethesda North Hospital, 10500 Montgomery Rd., Montgomery, OH 45242. Schedule: Full Time Day. Job Overview Assess the psychosocial needs of patients and families to develop, communicate, and implement an effective and comprehensive plan for psychosocial interventions and complex discharges, including readmissions in the high‑risk population. Be a resource regarding social systems, institutions, and behavioral and relationship patterns, and skillfully apply interventions to meet patient and family needs. Collaborate with the multidisciplinary team to provide efficient and appropriate care, optimize health status, and ensure smooth transitions between levels of care. Responsibilities Communicate verbally and in writing with the multidisciplinary team to advise on the plan and patient response. Develop and implement a plan of care based on assessment, including counseling or crisis intervention, and facilitate linkages to community resources and services. Revise the plan based on patient and family response. Provide on‑call services according to departmental policy, responding in a timely manner and providing appropriate follow‑up. Respond to referrals and assess psychosocial and complex discharge needs using data from the medical record, patient, multidisciplinary team, family, and other available resources. Apply root cause analysis to readmissions and work with the healthcare team, patient, and family to resolve issues. Requirements Bachelor's Degree in Social Science or Social Work (Required). Up to 1 year of experience in clinical healthcare community social work (Required). Payer rules for reimbursement knowledge. Community resources for patients with limited funds.
