Opportunity Highlights

• Schedule: Monday-Thursday 8:00am-4:30pm. Friday 8:00am-12:00pm with admin time 12:00pm-4:00pm
• Job Setting: Clinic, approximately 25 patients per day
• Types of Cases: Consults, established patients, Botox for migraines, EMGs, read EEGs
• Credentialing: 60-90 days

Minimum Requirements

• Board Certified NCCPA Certified (if PA), AANP or ANCC Certified (if NP)
• Certifications: ACLS, BLS
• Licensure: Active Arizona license required
