Specialty: Physical Therapist Discipline: Therapy Start Date: 08/31/2026 Duration: 13 weeks Hours per week: 40 hours per week Shift: 8 hours, days Shift: Days, 5x8hrs Hours: 40 hrs/wk Start Date: Aug 31, 2026 Length: 13 weeks Openings: 2 Assignment Overview American Traveler is seeking a Home Care Physical Therapist with home health and OASIS experience to provide one-on-one therapy in patients' homes. Home health setting providing one-on-one physical therapy care in patients' homes. Schedule is Tuesday through Saturday, 8am-5pm. 13-week assignment with 2 open positions. Guaranteed 36 hours per week. McKesson/Netsmart EMR used. Responsibilities Assess patient conditions and developing individualized care plans to maximize mobility and independence Collaborate with an interdisciplinary team to ensure patient needs are met Utilize a variety of therapeutic modalities to optimize patient care Candidates should have no requested time off during the assignment Requirements Active Vermont PT license required (license required if applicable) Minimum 1 year of experience required Home health experience required OASIS experience required, as candidate will case manage BLS certification required McKesson/Netsmart EMR experience strongly preferred Proficiency in standard therapy measurement techniques including ROM, strength, and balance testing required Pay Package Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
