Homecare – Physical Therapist (Full Time) Territory: Alliance/Louisville, Ohio Business Hours: Monday – Friday, 8:00 AM to 4:30 PM Benefits Flexible schedule Paid time off Minimal holiday rotation Hardship program to support staff during challenging times Patriot Cares nonprofit for community outreach Competitive salary with per diem earning potential for additional visits Mileage reimbursement for company travel Medical, dental, vision, and supplemental insurance starting immediately upon hire 401(k) with 4% company match Responsibilities Provide initial and ongoing comprehensive assessments of the patient’s needs, including Outcome and Assessment Information Set (OASIS) assessments Establish a treatment plan in consultation with the physician and revise it with the physician’s approval as necessary Assist in development and implementation of the interdisciplinary patient care plan as it pertains to physical therapy Assist the physician evaluating patients by applying diagnostic and prognostic muscle, nerve, joint, and functional ability tests Teach, supervise, and counsel the family and patient in the physical therapy program Write clinical/progress notes on the visit date and incorporate them into the patient record within 72 hours Job Conditions Ability to drive and access patients’ homes, which may not be routinely wheelchair accessible Hearing, eyesight, and physical dexterity sufficient to perform a physical assessment and patient care Occasional need to bend, stoop, reach, and move patient weight up to 250 pounds; lift or carry up to 30 pounds Qualifications Graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association Licensed or registered by the State At least one year of institutional/home health experience Completed a criminal background check Current CPR (BLS) certification Patriot Homecare is an Equal Opportunity Employer. #J-18808-Ljbffr
