Nurse Practitioner Geriatric Home Visits | Tucson AZ | Performance Bonus

An exciting opportunity to join a value-based care organization based in Arizona. The practice is seeking a board-certified Nurse Practitioner with a background in Primary Care/ Adult Medicine or Geriatrics to join their team.

The organization operates under a value-based care model emphasizing preventive medicine, chronic disease management, care coordination, and quality performance metrics. Nurse Practitioners practice in an outpatient setting supported by interdisciplinary clinical staff, with structured workflows designed to support patient panel management and continuity of care.

• Traveling role seeing patients in their homes.
• 6-8 patients per day, No Call, No nights, No Weekends.
• Medical Assistant is in clinic and assigned to you for full support.
• Geriatric Population, experience preferred but not required.
• Competitive Base Salary at $150,000K, performance bonus averaging $10,000K a year, 6 weeks of PTO, paid malpractice, full benefits and a matching 401K.
