The OT develops individualized plans of care designed to improve independence with activities of daily living, functional mobility, strength, coordination, balance, cognition, safety, and overall quality of life.Reporting: This position reports directly to the Facility Rehab Director.Why This Role MattersEvaluate patients according to physician orders, payer authorization, professional standards, and facility guidelinesDevelop individualized occupational therapy plans of care based on evaluation findings, patient needs, and functional goalsProvide appropriate occupational therapy interventions designed to improve activities of daily living, functional mobility, strength, coordination, balance, cognition, safety, and independenceEstablish measurable treatment goals focused on achieving optimal functional outcomesMonitor patient response to treatment and modify treatment plans and goals as clinically appropriateAssess patient performance with activities of daily living, transfers, mobility, positioning, coordination, and functional activitiesIdentify and recommend appropriate adaptive equipment, assistive devices, positioning strategies, and environmental modificationsScreen individuals who may benefit from Occupational Therapy servicesProvide patient, family, and caregiver education regarding treatment plans, progress, safety strategies, adaptive techniques, and functional outcomesCommunicate patient status, progress, and needs with patients, families, caregivers, physicians, nursing staff, and the interdisciplinary teamComplete accurate and timely evaluations, treatment notes, progress reports, discharge summaries, and required documentationEnter patient and treatment information accurately into EMR and rehabilitation systemsDirect and supervise Certified Occupational Therapy Assistants, rehabilitation aides, and students in accordance with applicable requirementsCoordinate treatment activities and goals with other members of the rehabilitation teamParticipate in interdisciplinary collaboration and rehabilitation programsMake appropriate referrals to other members of the rehabilitation and healthcare team when indicatedUse professional and clinical judgment to help ensure patient and staff safetyAttend required meetings, in services, training sessions, and educational presentationsPrepare and submit accurate patient charges and billing informationSupport quality assurance, department operations, and program developmentMaintain and enhance clinical and professional knowledge through continuing education and professional developmentMaintain valid licensure and required certificationsWhat We’re Looking ForCurrently completing, completed, or recently graduated from an approved Occupational Therapy program. Graduating students and new graduates are encouraged to applyMust have or be eligible to obtain the appropriate California Occupational Therapist license or authorization required to practice prior to providing Occupational Therapy servicesExperienced Occupational Therapists must maintain a current, active, and unencumbered California Occupational Therapist license as required for the positionCPR certification required.
