Social Services Director Job Summary: Westgate Hills Rehabilitation and Nursing Center, a skilled rehabilitation and nursing facility in Havertown, Pennsylvania, is seeking a dedicated and compassionate Social Services Director to join our team. Responsibilities: Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being Develop and implement written care plans for each resident's social services needs Hold meetings with patients/residents, families, and significant others to provide support and information to improve quality of life Perform patient/resident discharge planning activities, including family/patient education, nursing center documentation, and referrals to appropriate agencies and suppliers Establish and maintain relationships with community-based agencies and services Qualifications: Bachelor's degree from an accredited school of Social Work or related field required At least one year of supervised social work experience in a Skilled Nursing Facility, working directly with individuals, preferred Licensed in Social Work for the state of PA Benefits: Competitive rate Medical, Dental, Vision Benefits SOFI Wellness - Daily Pay - 401K Match - Tuition Reimbursement About Us: Westgate Hills Rehabilitation and Nursing Center is committed to providing Resident-Centered Care as part of our Rapid Recovery Program.
