GENERAL DESCRIPTION:

The Community Health Worker (CHW) serves as a vital link between communities and health outcomes. CHW’s provide in home visitation services to eligible families. They work to promote health and wellness, prevent disease, and improve overall well-being by providing culturally appropriate health education, support, advocacy, and resource navigation to underserved populations.

SUMMARY OF ESSENTIAL JOB FUNCTIONS:

• Conducts home visits in-person lasting at least 60 minutes. Virtual visits are completed per need and approved by supervisor; assesses family needs and provide case management services, when necessary, while promoting self-sufficiency.
• Maintains Community Health Worker Certification, 16hours of core Training, 12 hours of supervised home visits, 90% passing score on post-test within 90 days of hire.
• Required 16 CEU hours annually.
• Provides families with best practice individual services, connections to needed support, resources and referrals to improve family health and wellness, and resiliency outcomes.
• Conducts and participates in community outreach and public health education.
• Conducts developmental, hearing and vision screenings for children ages 0-5.
• Screens parents for Depression, Relationship Assessment, and ATOD Screening & Brief Intervention.
• Promotes healthy lifestyles, medical home and vaccines.
• Utilizes child development curriculum with each family and promote positive parent-child relationships.
• Maintains client and record confidentiality.
• Utilizes appropriate limit setting and boundaries.
• Engages in data collection, data entry into the prescribed data ETO System, and reporting.
• Maintains necessary assessments and documentation in accordance with agency and funding source requirements. All HF tools, plus Fetal Alcohol Screening, Family Planning Tool, the Needs Assessment, Pregnancy Testing.
• Participates in and comes prepared for bi-weekly supervision with Program Director.
• Coordinates with other community services.
• Attends and participates in all scheduled agency, funder or program conferences, community events, seminars, in-service training, meetings and parent events. Attend monthly meetings with the mental health and nursing consults.
• Maintains all Health Start quality assurance standards.
• Participates in agency, model and or funder Continuous Quality Improvement activities. Will be required to attend monthly MIECHV CQI meetings (one with MIECHV and one with agency staff) and participate in the CQI goal requirements.
• Perform other related duties as assigned.

REQUIREMENTS:

• Associates/Bachelor ’s Degree in Social Work or related field preferred or High School Diploma and two years of experience in related field.
• Ability to interact effectively with families.
• Knowledge in family life skills and Child Development.
• Experience in working with at-risk children and their families.
• Knowledge of and sensitivity to cultural differences.
• Ability to communicate effectively both orally and in writing.
• Ability to work effectively on a family support team.
• Flexibility in scheduling to meet the needs of program participants (to include evening or weekend visits). Please initial to confirm understanding of statement regarding flexibility in scheduling.
• Adhere to the schedule agreed upon with the supervisor.
• Bilingual Spanish preferred.
• Must be eligible and able to obtain/maintain a level 1 fingerprint clearance card.
• Must be certified in CPR/First Aid.
• Location: must be able to provide direct service to the designated service area.
• Maintain a functional and safe home working space with adequate internet connection for the job.
• Comply with agency and program policies and procedures including confidentiality, employee conduct, computer usage and dress code.
• Meet program/department’s performance and productivity standards. First 90 days are strictly in office, followed by a 30 day ‘work from home trial’ phase. Working remotely depends on satisfactory performance and productivity.
• Attend required agency and program/department meetings.
• Follow and model CFR’s core values.
• Participate in PQI initiatives.
• Must have a valid Arizona driver’s license, vehicle and meet agency liability insurance requirements.

ABILITY REQUIREMENTS:

• Sitting for extended periods of time daily; standing and walking short distances.
• Moderate twisting, bending, stooping and lifting up to 40 pounds in the performance of assigned duties.
• Normal manual dexterity and eye-hand coordination required.
• Repeated hand-wrist movement required.
• Ability to use a variety of office equipment and machines as referenced.

The nature of the position involves a fast-paced working environment with multitasking of many different duties and tasks. General daily priorities may change at a moment’s notice, and the position requires quick response time and flexibility.
