LTSS Service Care Manager

Position Title: LTSS service care manager
Work Location : This position will be visiting member homes. Please source anywhere within Delaware. Kindly indicate in the resume which county the candidate is residing/can support Assignment Duration : 6 months Work Arrangement : Field-based (member home visits)Position Summary : Position will be completing LTSS level of care redetermination and will be completing a mixture of care visits and supporting other LTSS case managers.
Background & Context: Support the case management team due to high increase in membership.

Required: Bachelor's degree and 2 - 4 years of related experience.
