Responsibilities
- Assess patient needs and develop individualized care plans with clearly defined goals.
- Coordinate with an interdisciplinary team to implement and adjust plans of care.
- Monitor patient status, manage the progression of the caseload, and address barriers to goals.
- Educate patients and families about care plans, medications, symptom management, and follow-up steps.
- Document clinical assessments, plans, and outcomes in the electronic health record; communicate with referring providers and care partners.
- Facilitate referrals, referrals to ancillary services, and appropriate post-visit resources to support safe transitions.

Requirements
- Active Registered Nurse (RN) license (state-specific or as required for practice).
- Relevant case management and clinical experience in an outpatient setting.
- Strong assessment, critical thinking, and communication skills.
- Ability to collaborate with an interdisciplinary team and coordinate care across services.
- Current CPR/BLS certification or ability to obtain.
- Education: RN degree (Associate Degree in Nursing or higher) as the foundational credential.
