Contract Case Manager - Behavioral Health & School-Based

Deblin Health Concepts in the Greater Houston Area seeks a Contract Case Manager to support LOC 3/4 clients, including school-based services, with local travel between campuses. You will assess, develop and monitor treatment plans with clinicians, coordinate care, connect clients to resources, and document progress.
