Physical Therapist (PT) - Must Be Licensed

Job Overview A great SNF / LTC seeks an experienced or newly graduated Physical Therapist / PT for a contract assignment. Responsibilities Assess referred patients requiring rehabilitation services, including direct patient care to evaluate medical condition, functional capabilities, limitations, restrictions, and potential for rehabilitation. Establish and administer treatment programs with specific goals determined by the patient’s capacity and tolerance under physician direction. Set realistic, achievable goals; document and record patient condition; educate patients and families on appropriate physical therapy methods. Develop a plan of care for each patient and provide skilled physical therapy services/interventions per physician orders. Assess patient needs, plan, evaluate, and modify care to meet therapy goals. Collaborate with all disciplines to plan and evaluate team goals for each patient. Qualifications Graduate from an accredited school. Current state license as a Registered Physical Therapist / PT or actively pursuing licensure. Current CPR certification. Experience in plan-of-care development and skilled PT service delivery.
