WHAT YOU'LL DO

Conduct telephonic outreach to patients and/or family members to assess for disease exacerbations Collect and document comprehensive clinical history Assess patients for exacerbations of chronic conditions including CHF, COPD, diabetes, dehydration, and others as needed Interpret electronic patient dashboards to support clinical decision-making Notify the patient's PCP and relevant third partiesin the event of an exacerbation Document all assessments electronically ina timelyandaccuratemanner Submit timesheets on time through theCareLinxapp Communicate with your Care Advisorregardingany case updates or changes in patient condition Maintain client confidentiality at all times

WHAT WE'RE LOOKING FOR

Required Qualifications: Active RN license in California and Utah; licensure in additionalstates preferred Bilingual fluency in English and Spanishrequired Minimum 2 years of clinical experience in one or more of the following: Acute Care ED, ICU, Telemetry, Stepdown/Progressive Care Unit Experience conducting clinical history and assessments with older or geriatric patients Access to a smartphone capable of running theCareLinxapp (requiredfor scheduling, documentation, and timesheets) Must pass a background check Skills & Attributes: Compassionate and professional — able to build trust quickly over the phone Reliable — when you commit to a shift, you show up on time, every time Strong clinical judgment and ability to prioritize in a fast-paced telephonic environment Detail-oriented documentation skills —accurate,timely, and thorough Tech-comfortable — confident navigating electronic dashboards and documentation systems
