Through trusted relationships and community-based support, we partner with health plans and local organizations to help members navigate healthcare, stay engaged in care, and live healthier lives.We are launching a new clinical pilot that integrates physicians, care navigators, and community support teams to create a more coordinated and human approach to care.About the RoleWe are seeking a South Carolina-licensed physician to serve as a consulting clinician for an early-stage care program. In this role, you will conduct initial virtual visits with members to understand their medical needs, barriers to care, and overall health goals.Your evaluation helps set the direction for the member's care journey. After the initial visit, ongoing support is provided by a care team including Community Health Workers and Licensed Clinical Social Workers, allowing you to focus on clinical guidance rather than longitudinal case management.This role is ideal for physicians interested in innovative care delivery, telehealth, and population health models that extend beyond traditional office-based practice.What You'll DoConduct initial telehealth visits with enrolled membersAssess medical needs, care gaps, and barriers to accessing careEstablish a clear clinical plan and care prioritiesCollaborate with care team members to ensure a smooth handoff and continuity of careProvide clinical insight to help refine program workflows during the pilot phaseSupport a patient-centered approach addressing social and medical needs togetherWhat You BringMD or DO with an active, unrestricted South Carolina license. Multiple state licenses strongly preferred.Board certified or board eligible, with a primary care specialty background in Family Medicine, Internal Medicine, or a related primary care specialty preferred5+ years of physician experience in a non-emergency setting2+ years of experience caring for Medicare, Medicaid, or other complex populationsExperience with care coordination, health navigation, or whole-person care modelsExperience collaborating with care navigators, community health workers, licensed clinical social workers, nurses, and leadershipAbility to build trust quickly with patients in a virtual care settingTelehealth experience preferredExperience with Google Suite preferredExperience with Healthie preferred1099 independent contractorPart-time, flexible schedulingVirtual visitsPilot program with opportunity to expand as the model growsWhy This Role Is DifferentThis is not a traditional panel-based practice.You focus on understanding the patient and setting direction — while a dedicated support team helps carry out the ongoing engagement.
