Reports to: Case Management Team Manager
A Community Health Worker (CHW) is a frontline public health worker who is a part of the community they serve. Drawing from lived experience, CHWs are trusted members of the care team who support the health and wellness goals of individuals, families, and communities. The Community Health Worker works within SLMH's Case Management Team and provides support for clients while seeking medical care, and works closely with medical providers, primary care teams, and other agencies, as well as SLMH’s mental health treatment teams and the client to improve patient care and outcomes.
Duties and Responsibilities :
Care Coordination and Service Provision
- Build positive and supportive relationships with clients. Give helpful feedback and encourage them to take an active role in their health.
- Help clients get the health services they need. This includes finding a primary care provider, explaining how to use medical services, and helping remove barriers to care and social services.
- Provide one-on-one therapeutic support using appropriate mental health strategies to address symptoms and build skills.
- Check each client’s needs and progress based on the client's individual treatment plan.
- Help clients use available resources. This includes scheduling appointments and assisting with applications for programs they may qualify for.
- Maintain strong working relationships with clients, healthcare providers, agency partners, supervisors, and office staff.
- Communication between clients and providers, and attend appointments with clients when needed. Work with medical providers to reduce cultural and financial barriers. Help ensure clear
- Support clients in creating health goals and care plans. Follow up regularly with both clients and providers to track progress.
- Help clients understand their care plans and instructions. Coach them on managing chronic conditions and practicing self-care.
- Provide outreach activities and supports to potential clients.
Complete administrative and continuing education responsibilities
- Schedule appointments with assigned clients, and coordinate and maintain appointment calendar.
- Complete all required paperwork, including notes on each session and individual client paperwork, in a timely manner.
- Meet requirements regarding productivity and dependability.
- Coordinate with co-workers and treatment teams, including participating in complex case reviews or family systems reviews as necessary.
- Actively participate in clinical supervision, team meetings, and staff meetings.
- Engage in continuing education activities and trainings, and continually build skills.Participate in completeness and quality reviews as required.
- As required, positively represent SLMH in the community and with other service providers,and/or act as a liaison with other organizations or with internal committees/cross-departmental teams.
- Perform all duties in a respectful and responsible manner, both with clients and coworkers, and in a manner that meets all professional ethical standards.

Qualifications :
E ducation and Experience:
- Certification as a Community Health Worker and inclusion on the Traditional Health Worker Registry is required.
- Preferred: At least 3 years of active employment in the field of mental health service provision.
- Preferred: Ability to be credentialed as a QMHA .
- Preferred: Experience working in a mental health setting.

- Strong attention to detail and organizational skills.
- Ability to ask for assistance and utilize feedback effectively.
- Ability to remain calm and professional in stressful situations
- Ability to balance clinical, cultural, and ethical values, particularly as relates to client and staff needs.
- Strong written and oral communications skills.
- Ability to interact positively with people of all ages and cultural background.
- Ability to work both independently and as part of a collaborative team environment.
- Sound computer skills.
Working conditions and physical requirements:
- This is a physically active role, located in partially an office environment and partially in many community locations, including client homes, and requires regular interaction and engagement with clients. The employee is regularly required to talk or hear, walk, stand, communicate through speech, and use tools or controls. Occasional need to lift and/or move up to 25 pounds.
- Utilizing a personal cell phone will be required; SLMH provides applications to ensure secure communication. SLMH will provide other technology tools including laptop and printer, etc.
- Valid Driver’s License and reliable vehicle with active insurance are required ; travel between offices, client homes and community locations required multiple times per day and ability to respond to locations up to 30 miles a way in a timely fashion required. A DMV report will be performed for this position.
