As a Community Health Worker, you will serve as a trusted connection between patients, healthcare providers, and community-based resources. This role supports individuals and families in improving overall health outcomes by reducing barriers to care, providing health education, coordinating services, and promoting access to medical, behavioral health, and social support services. Community Health Workers play an important role in strengthening patient engagement, improving health equity, and supporting long-term wellness through outreach, advocacy, and education.
For Foster Care-focused positions, the Community Health Worker supports children involved in foster care, children adopted from foster care, youth involved with the Division of Youth Services or Juvenile Justice systems, and their families. This role helps ensure access to medical, dental, behavioral health, and social support services while coordinating care and strengthening connections between families and community resources.

Key Responsibilities
- Facilitate communication between patients, families, healthcare providers, and social service systems
- Support patient empowerment by helping individuals navigate healthcare and community resources
- Identify and troubleshoot barriers impacting access to care, treatment adherence, and healthcare goals
- Engage in community outreach to recruit and connect with high-risk or underserved populations
- Serve as a liaison between healthcare teams, behavioral health providers, and community organizations
- Assist care teams with care coordination, disease management, patient education, and self-management strategies
- Connect individuals and families with community-based resources and internal health center services
- Conduct outreach through phone calls, home visits, and visits in community settings
- Collaborate with patients and care teams to develop solutions that support health and wellness goals
- Conduct community education and awareness activities related to available healthcare and support services
- Provide fair and impartial education regarding Qualified Health Plan enrollment and assist with eligibility and enrollment processes when applicable
- Provide services in a culturally and linguistically appropriate manner for the populations served
- Complete documentation accurately and timely according to agency standards
- Participate in assigned projects and support department initiatives
- Perform additional duties as assigned
Foster Care-Focused Responsibilities (when applicable):
- Assist foster parents and caregivers in accessing medical, dental, and behavioral healthcare for children in care
- Support required initial and follow-up healthcare visits for children in foster care
- Complete PRAPARE and Social Determinants of Health (SDOH) screenings annually or when placement changes occur
- Coordinate referrals and follow-up care for children and foster families
- Connect foster families with community-based supports and resources to meet social and healthcare needs
- Support continuity of care and care transitions for foster youth and families
Education and/or Experience Qualifications
Community Health Worker:
- High School Diploma or equivalent required
- Bachelor's degree from an accredited college or university in Social Work, Psychology, Public Health, Nursing, Health Education, or related field preferred
- Two (2) years of experience in community health, healthcare outreach, behavioral health, or human services preferred
- Preferred experience may include:

- Public speaking
- Data entry
Community Health Worker – Foster Care Preferred Experience:

- Individuals who were involved in foster care
- Foster or adoptive parents
- Individuals who adopted through foster care
- Experience working or volunteering with child welfare, child advocacy, juvenile justice, or behavioral health organizations
Required License/Certification (one of the following):
- Valid driver's license in state of residence
- Community Health Worker certification preferred where applicable
- CHW training through a recognized curriculum provider preferred for Foster Care-focused roles

- Knowledge of community resources and social support systems
- Understanding of local community culture and values
- Strong customer service and relationship-building skills
- Ability to advocate for individuals and families in a compassionate and professional manner
- Excellent verbal and written communication skills
- Strong organizational and time-management skills
- Ability to work independently while collaborating effectively with multidisciplinary teams
- Proficiency with Microsoft Office and/or Google platforms
- Ability to maintain confidentiality and professional boundaries
- Commitment to culturally responsive, trauma-informed care
Foster Care Additional Qualifications:
- Knowledge of foster care systems and child welfare services preferred
- Trauma-informed approach when supporting children and caregivers
- Understanding of behavioral health conditions affecting youth
- Familiarity with social determinants of health and community support systems
Supervisory Requirements:
- None
Employment Requirements:
- Must provide evidence of freedom from infectious and contagious disease prior to employment and annually thereafter, when applicable
- TB screening required for direct care staff; mandatory for designated Kansas programs
- Annual Health Assessment for designated programs
- Successful completion of background check including criminal record, driving record, abuse/neglect, and fingerprint checks
- Completion of New Hire Orientation at the start of employment
- Completion of all required Relias and annual trainings
- Current driver's license, acceptable driving record, and current auto insurance required
- First Aid certification within three (3) months of employment and upon renewal
- CPR certification within three (3) months of employment and upon renewal
- Behavioral Management training within thirty (30) days or first available training:
- CPI for Residential programs
- Peaceful Intervention or Verbal De-escalation for Outpatient programs
- Completion of Direct Service Expectations within thirty (30) days of employment
- Maintain knowledge of policies, procedures, and regulations related to assigned program
- Foster Care positions may require:
