Nurse Case Manager

Responsibilities In addition to managing complex cases, you will serve as a mentor and resource for developing case management professionals, helping to strengthen team performance and foster a culture of learning, collaboration, and excellence. The successful candidate will bring strong clinical judgment and exceptional communication skills, and will have the opportunity to independently manage challenging situations, evaluate patient needs, coordinate interdisciplinary services, and contribute to solutions that improve both individual and organizational outcomes. Beyond traditional care coordination, you will help patients overcome barriers to healthcare access, understand their treatment options, and successfully navigate available resources. Qualifications Qualified candidates must be licensed Registered Nurses with a minimum of one year of paid experience as an ADvantage Case Manager and at least one year of paid supervisory or personnel management experience. Candidates should demonstrate a history of leadership, sound clinical decision-making, and a commitment to delivering compassionate, high-quality care. Additionally, one year of paid professional experience with the aging adult or disabled population is required.
