RN, Care Manager (Home Health)

We’re seeking a dedicated and independent Home Health Registered Nurse (RN) to join a patient-centered healthcare team serving communities across California, Hawaii, Oregon, and Washington. What You’ll Do: Conduct comprehensive patient assessments and develop individualized care plans Provide skilled nursing care and ongoing evaluations in patients’ homes Educate patients and caregivers on treatment plans, medications, and preventive care Collaborate with interdisciplinary healthcare teams to ensure coordinated patient care Maintain accurate and timely clinical documentation in compliance with agency standards Monitor patient progress and adjust care plans as needed Participate in on-call rotations as assigned What You’ll Bring: Active Registered Nurse (RN) license in the state of practice CPR/BLS certification required Valid driver’s license, reliable transportation, and current automobile insurance required Home health or acute care nursing experience preferred Strong clinical judgment, communication, and patient education skills Ability to work independently while managing a patient caseload effectively #J-18808-Ljbffr
