LSW Case Manager

Position: LSW Case Manager Location: Cleveland, Ohio Compensation Base salary: $61,547.20 per year. New hire bonus: $5,000. Total compensation: $66,547.20. Position Summary As an LSW Case Manager, you will support long‑term clients age 60 and older enrolled in the MyCare Ohio CareSource waiver programs through comprehensive assessment, care coordination, advocacy, and ongoing case management. This opportunity combines community impact, relationship‑driven service, and hybrid field‑based flexibility while helping individuals remain in the place they call home.
