Patient Navigator/Medical Assistant (MA) - Long COVID

$25.04 - $33.11 per hour

Position Summary The Patient Navigator will work collaboratively with patients and all team members to navigate the health care system so that care can be delivered effectively based on the individual patient’s needs and assets. The Patient Navigator will be a point of contact for the clinic and will work with National Jewish Health, referring entities, CO-MTN (Colorado Multidisciplinary Translation Network) and patients to seamlessly coordinate care. The Patient Navigator will assist patients with connecting to available resources and will support them during clinic visits. Responsibilities Obtain, organize, and maintain patient information, clinical forms, diagnostic results, and other necessary information to ensure it is available at the time of appointment. Coordinate activities and flow of work for the Long Covid Clinic. Serve as a resource to staff regarding departmental policies, procedures, and services. Act as one of the first points of contact for the clinic, preparing and completing all required documentation accurately. Collaborate with National Jewish Health staff and referring clinic staff to provide seamless care and care transitions; facilitate referral process and exchange of patient‑related medical records, and assist in obtaining release‑of‑information forms. Orient patients to the clinic upon visit. Participate in pre‑clinic huddles to prepare for clinics and facilitate communication and coordination among health care team members. Assist with efficiency in clinic flow and patient visits, anticipating and planning for potential problems or issues. Maintain supplies in exam rooms and laboratory supplies to ensure uninterrupted service. Perform all assigned diagnostic procedures in accordance with ATS and departmental guidelines, documenting services and other care‑related items. Work with outside vendors and patients to address equipment and customer service issues; assist in providing proper documentation to DME companies to facilitate patients’ DME needs. Utilize follow‑up system to assure care follow‑up and coordination is accomplished. Collaborate with multidisciplinary teams to assist with problem identification and resolution, cost containment issues, implementation of new services, and systems/performance measures. Facilitate the development, implementation, and evaluation of program or area services and initiatives. Facilitate the development of practice standards and measurable outcomes based on performance evaluation and research. Perform competencies, including age‑specific competencies, as identified through the departmental competency program. Adhere to state and federal regulations governing disclosure of private health information. Participate in the development, data collection, and analysis of team‑based quality improvement projects. Support research and quality improvement projects that contribute to improving patient care, staff development, and the mission of the institution. Minimum Qualifications Education: High school graduate or equivalent preferred. Graduate of an accredited Medical Assistant program or other medical program that meets the criteria of a medical assistant program or successful completion of an approved nurse aide training program / Certified Nursing Assistant Program. Work Experience: 1 year of experience as a Medical Assistant or Certified Nursing Assistant required. Special Training, Certification or Licensure: CPR/BLS required; Medical Assistant or CNA certification required.
