About the Role:

As a Physical Therapist PT with our team, you will play a vital role in helping our residents recover from a variety of conditions, including cardiac, orthopedic, renal, and pulmonary issues. You will have the opportunity to work in a dynamic and supportive environment, collaborating with other healthcare professionals to develop and implement personalized therapy plans.

Responsibilities:

• Conduct initial and ongoing assessments of residents' physical function and ability to perform daily activities

• Develop and implement individualized therapy plans to address residents' needs and goals

• Provide physical therapy services, including exercises, stretches, and other therapies as needed

• Document resident progress and maintain accurate records

• Communicate with residents, families, and other healthcare professionals to ensure seamless care transition

Requirements:

• Bachelor's or Master's degree in Physical Therapy from an accredited program

• Current Physical Therapy license in the State of New Jersey

• Strong clinical skills and ability to work effectively with residents with complex conditions
