Responsibilities
- Provide home health care to patients, ensuring high-quality care.
- Develop and implement personalized care plans.
- Educate patients and their families on home health care and treatment plans.
- Monitor and document patient progress, adjusting care plans as needed.
- Collaborate with other healthcare professionals to ensure comprehensive care.
Requirements
- Certification: Licensed Registered Nurse (RN) with no restrictions.
- Experience: At least 1 year in a similar role is highly preferred.
- Skills:
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a team.
- Proficiency in medical documentation.
