Nurse Practitioner (NP) / Physician Assistant (PA) — Nursing Home Primary Care | Brooklyn, NY (Canarsie) Location: Brooklyn, NY (Canarsie) Compensation: $500 – $600 per day (1099) Schedule: Full-Time | Monday–Friday | Availability between 8:00 AM – 6:00 PM A medical group providing primary care services to nursing homes is seeking a Nurse Practitioner or Physician Assistant to provide ongoing coverage for a skilled nursing facility located in the Canarsie section of Brooklyn. This role focuses on providing primary care to geriatric patients in a nursing home environment. The provider will manage patient visits, follow‑ups, and coordinate care with facility staff to ensure continuity of care. Position Overview The provider will be responsible for managing a panel of nursing home patients, addressing both routine and acute medical needs while ensuring high‑quality, coordinated care in collaboration with facility staff. Clinical Responsibilities Provide primary care services to geriatric nursing home residents Conduct routine and urgent patient visits Manage chronic conditions and acute medical issues Coordinate care with nursing staff and facility leadership Participate in CCM and RPM programs when applicable Document patient encounters in facility EMR systems (SigmaCare or PCC) Requirements Active New York NP or PA license Active Medicare and Medicaid provider ID Experience working with geriatric patients preferred Skilled Nursing Facility experience preferred RN background in a nursing home or long‑term care setting is highly valued What We Offer Competitive daily compensation ($500–$600/day) Stable full‑time schedule Consistent full‑time patient panel and coverage Supportive clinical and administrative team #J-18808-Ljbffr
