Total Compensation: Up to $220,000/year

• Base Salary: $145,000/year
• Performance Incentive: Up to $75,000/year
• Retention Bonus: $10,000 after 1-year anniversary

Job Summary

We are seeking a compassionate and experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our ACO REACH High Needs House Calls Program . In this role, you will provide comprehensive in-home primary care to medically complex and high-risk patients while helping reduce hospitalizations and improve overall patient outcomes. You will collaborate with an interdisciplinary healthcare team to deliver personalized, value-based care focused on improving quality of life.

Key Responsibilities

• Provide comprehensive primary care services to homebound and high-needs patients.
• Perform physical assessments, obtain medical histories, and develop individualized treatment plans.
• Diagnose and manage acute and chronic medical conditions.
• Order, interpret, and review laboratory tests, diagnostic studies, and imaging.
• Prescribe medications, treatments, vaccinations, and other therapeutic interventions.
• Manage chronic diseases, pressure ulcers, wounds, Foley and suprapubic catheters, and perform phlebotomy/IV access when needed.
• Educate patients and caregivers on disease prevention, medication management, nutrition, and healthy lifestyle practices.
• Collaborate with physicians, nurses, care managers, specialists, and hospital-at-home teams to coordinate patient care.
• Participate in interdisciplinary case conferences and patient care reviews.
• Maintain accurate EMR documentation, coding, and quality reporting in compliance with ACO standards.
• Participate in a rotating after-hours emergency call schedule.
• Work alongside Primary Care Physicians and specialists for continued clinical development and mentorship.
• Support patients with social determinants of health through collaboration with care management and Health Home programs.

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Current New York State NP or PA license.
• National Board Certification.
• Active DEA license.
• Minimum of 3 years of clinical experience , preferably in home-based primary care, geriatrics, or chronic disease management.
• Experience with value-based care models, ACOs, Medicare programs, or risk-based care preferred.
• Strong knowledge of EMR/EHR systems.
• Bilingual in Spanish is strongly preferred.
• Ability to work independently while collaborating with a multidisciplinary healthcare team.
• Excellent communication, clinical decision-making, and patient-centered care skills.
