Serving patients across 50+ assisted living facilities Experiencing significant growth with strong provider retention and high satisfaction rates Focused mission: Keep patients healthy and out of the hospital Benefits and Features $70/hour — approximately $145,000 annually 20 Days PTO $10,000/year Health Insurance Reimbursement Role Providers conduct video visits from a clean, well-supported clinic environment in Brooklyn. On-site Medical Assistants and med techs at the assisted living facilities assist with vitals and exam preparation, allowing you to focus on clinical decision-making — without the noise and unpredictability of traditional facility rounding. Full-time, Monday–Friday, no nights, no weekends Work out of the Brooklyn clinic conducting video visits with patients in assisted living facilities Medical Assistants on-site perform vitals and assist with coordination Goal is proactive management of geriatric patients to prevent hospitalizations This is an ideal role for a provider who enjoys geriatric medicine and wants high-impact patient care in a structured, efficient setting Background Desired Licensed Nurse Practitioner or Physician Assistant in New York Active DEA License Must already be credentialed with Medicare and NY State Medicaid Why This Role?
